LIBRE SUPPORT PROGRAM TERMS AND CONDITIONS

Abbott sponsors the program to facilitate getting started with Your Continuous Glucose Monitor (“CGM”) system to manage Your diabetes. References in these terms and conditions (“Terms”) to the “Program” means the Libre Support Program, which is managed by IQVIA on behalf of Abbott. References to “You” or “Your” in these Terms are references to the individual enrolling in the Program. Personal information collected will be used for enrollment and eligibility approval and may be used to perform analytics to improve the service provided to You. Your personal information may be shared with Abbott and with third parties only where necessary to fulfill the purposes of the Program. This may include reaching out to You via email and/or phone to provide support related to the Program. For those who voluntarily provided a fax number and expressly agreed to receive faxes, Abbott and/or its affiliated entities may also contact You by fax at the number(s) You provided regarding the Program and the status of Your patient(s)’ participation in the Program. You may opt-out of receiving these faxes at any time by contacting the following toll-free telephone number 1-833-855-4273, toll-free fax number 1-800-353-6498, and/or by completing a form at https://getstartedlibresupport.opushealth.com/faxunsubscribe, 24 hours a day, 7 days a week, 365 days a year. The Program enables access to Certified Diabetes Care and Education Specialists to provide education and support as You get started on your CGM. If You disagree with these Terms (as they may be amended from time to time), or are dissatisfied with this Program, You may end Your participation in the Libre Support Program at any time. Eligibility Criteria:

This Program is available for patients with Type 1, Type 2, or gestational diabetes and/or caregivers supporting these patients. Patients ages 2-17 are eligible for the Program through their parent or guardian. The Program is not provided as any inducement for future purchases. This Program is void where prohibited by law. Abbott may modify or rescind this Program at any time without notice.

SMS TERMS AND CONDITIONS

By providing IQVIA with a mobile number and providing consent for text and short code (SMS) messaging from the Program, You are verifying that it is Your mobile number and that You are providing prior written consent to the Program to receive text and SMS messaging so the Program may communicate with You and provide general informational messages and promotional marketing messages to You on Your cell phone. You understand and agree that You may receive these messages through the use of automated technology. Message and data rates may apply. To opt-out of receiving SMS messages, reply “STOP” to any message or text “STOP” to the short code provided. After You send the SMS message “STOP” to us, we will send You an SMS message to confirm that You have been unsubscribed. After this, You will no longer receive SMS messages from us. If You would like to opt back in to receiving SMS messages, please call 1-833-855-4273 to opt in again. After we receive Your opt-in request, we will start sending SMS messages to You again. If You are experiencing issues with the messaging program, You can get help by calling our toll-free number, 1-833-855-4273. Message frequency may vary. If You change Your mobile phone service provider, the SMS messaging service may be deactivated and You will need to re-enroll in the notification service. IQVIA may change or end the messaging program at its discretion, and the content may not be available on all wireless carriers. IQVIA may add or remove its capabilities with certain wireless carriers at its sole discretion and at any time without any notification to You. Participating carriers may include, but not be limited to: AT&T, Verizon Wireless, Sprint, T-Mobile, MetroPCS, U.S. Cellular, Alltel, Boost Mobile, Nextel, Virgin Mobile, Alaska Communications Systems (ACS), Appalachian Wireless (EKN), Bluegrass Cellular, Cellular One of East Central IL (ECIT), Cellular One of Northeast Pennsylvania, Cincinnati Bell Wireless, Cricket, Coral Wireless (Mobi PCS), COX, Cross, Element Mobile (Flat Wireless), Epic Touch (Elkhart Telephone), GCI, Golden State, Hawkeye (Chat Mobility), Hawkeye (NW Missouri), Illinois Valley Cellular, Inland Cellular, iWireless (Iowa Wireless), Keystone Wireless (Immix Wireless/PC Man), Mosaic (Consolidated or CTC Telecom), Nex-Tech Wireless, NTelos, Panhandle Communications, Pioneer, Plateau (Texas RSA 3 Ltd), Revol, RINA, Simmetry (TMP Corporation), Thumb Cellular, Union Wireless, United Wireless, Viaero Wireless, and West Central (WCC or 5 Star Wireless), and usage is subject to their respective terms of service. Your receipt of SMS messages may result in You incurring additional messaging or data fees from Your wireless carrier for which You are solely liable. You can contact Your wireless carrier with any questions about Your text or data plan. IQVIA and Abbott are not liable for delayed or undelivered messages.

The FreeStyle Libre 3 system and FreeStyle Libre 2 system are indicated for use in people with diabetes age 4 and older.

FreeStyle Libre 2 Plus sensor and 3 Plus sensor are indicated for use in people with diabetes age 2 and older.
